Forms on this website do not constitute a HIPAA-secured channel and should not be used to share clinical information. After we receive your initial contact, we will move all clinical communication to a secure channel that meets HIPAA requirements.

If you have questions about this notice, or believe your privacy rights have been violated, please contact our Privacy Officer at the practice address or main number. You will not be retaliated against for filing a complaint.
